Medical Information

On your child's admission form to Field Place, you will have noted any medical condition. Please notify the school if this changes in any way. If your child suffers from asthma or needs an inhaler in school a separate form is required.

For health and safety reasons, medicines in school are kept to a minimum. There may be a time when your child will need to have medicine during the school day e.g. Antibiotics. Following our medicines in school policy, staff will only administer medicines during the school day that are required 4 times a day or more, have been prescribed, and are presented in the original package with pharmacy labels attached.

You will need to bring the medicine into school yourself and speak to staff in the front office, where you will need to complete the necessary form. Please do not ask the class teacher to administer medicine, there are unable to do this.

You will notice a section in the New Entrants pack regarding Calpol and Piriton. If you choose to give your consent for these medicines, we will of course ALWAYS contact you first and a record will be kept.

All forms are available from the school office or can be downloaded from the link below. If you have any queries about medication, please do not hesitate to contact on 01903 700234 or email me at ledmunds@fieldplace.org.uk.
